Plan & Decide

Risk Register

Name the handful of things that could genuinely derail the project, give each a real owner and a real mitigation, and prune the list every week.

What’s inside

  • Read Me. Five steps to set it up and the habit that keeps it alive.
  • Register. ID, risk, category, likelihood, impact, owner, mitigation. The score fills itself in.
  • Reference. What each score means, the status lifecycle and mitigation strategies.
  • Worked example. A filled-in register from a login-migration project.

How to use it

  1. One row per risk, each with a stable ID.
  2. Set likelihood and impact. Score is L × I, from 1 to 9.
  3. One named owner and a real mitigation per risk, not "monitor closely".
  4. Move status as things change, Open, Mitigated, Closed or Realized.
  5. Review the top scores weekly and prune stale rows.
